Gum disease (periodontal disease) is one of the most prevalent oral health conditions among adults. It begins when plaque — a soft film of bacteria — accumulates along and below the gum line and is not fully removed through brushing and flossing. Over time, plaque hardens into tartar (calculus), which cannot be removed at home and requires professional care.
Left unaddressed, gum disease can progress from mild inflammation of the gums (gingivitis) to a deeper infection involving the bone and tissues that support your teeth (periodontitis). The encouraging news is that with appropriate professional care, many patients experience meaningful improvement in their gum health.
The earliest stage — the gums are inflamed, may bleed when brushed, and may appear red or swollen. At this stage, the bone and connective tissue have not yet been affected. With proper professional cleaning and improved home care, gingivitis is reversible.
The infection has begun to affect the bone supporting the teeth. Pockets between the gum and tooth have deepened. A deep cleaning is typically recommended at this stage to remove the bacteria and tartar below the gum line that a routine cleaning cannot reach.
Deeper pockets and more significant bone loss. Teeth may begin to feel sensitive. Deep cleaning may be performed in multiple sections, and closer monitoring at follow-up visits is typically recommended.
Significant bone involvement; teeth may feel loose. In some cases, a referral to a periodontist (gum specialist) is appropriate. Dr. Maddipati is straightforward about when a specialist referral would serve you best.
The stages above are general guidance. Only a clinical examination with periodontal probing measurements and X-rays can determine the extent of gum disease in your individual situation. Dr. Maddipati reviews her findings with you — including probe depths and X-ray images — before recommending any treatment.
Research has documented associations between periodontal disease and several systemic health conditions. Dr. Maddipati's background in public health means she considers these connections as part of her approach to your care — not just as an isolated dental issue.
There is a well-documented bidirectional relationship between diabetes and gum disease. Diabetes can make it harder for the body to fight infection, which may worsen gum disease. Conversely, chronic gum inflammation may make it more difficult to maintain stable blood sugar. Patients managing diabetes and gum disease together may benefit from coordinated care between their dental and medical providers.
Hormonal changes during pregnancy can intensify the gum tissue's response to plaque, making gum inflammation more pronounced. Maintaining good gum health during pregnancy is an important part of overall prenatal wellness. If you are pregnant and notice changes in your gums, an evaluation is a positive step.
Research has identified associations between periodontal disease and cardiovascular conditions. While the nature of this relationship is an active area of study, maintaining healthy gums is part of a broader picture of overall health. If you have cardiovascular concerns, your dental provider and physician can work together as part of your care team.
Bacteria in the mouth can be inhaled into the lungs. Some research suggests associations between periodontal disease and respiratory conditions, particularly in individuals who are already at elevated risk. Good oral hygiene and regular professional care are supportive of respiratory health as well.
The connections described above are based on current research and are intended to illustrate why gum health matters beyond the mouth. They do not imply that treating gum disease will treat or prevent any systemic condition. Please consult with your physician for guidance specific to your health situation.
A periodontal deep cleaning — also called scaling and root planing (SRP) — is a non-surgical procedure that addresses the plaque, tartar, and bacteria that have accumulated below the gum line. It goes beyond what a routine cleaning can accomplish and is specifically designed to treat active gum disease.
A routine dental cleaning (prophylaxis) removes plaque and tartar from the visible surfaces of teeth and just at the gum line — it is a preventive service for patients with healthy or minimally inflamed gums. A deep cleaning goes further below the gum line into the pockets that form when gum disease is present. It is a therapeutic treatment for active infection, not just maintenance.
Dr. Maddipati uses a small, graduated probe to gently measure the depth of the pockets around each tooth. She reviews these measurements with you — healthy pockets are shallow; deeper pockets with bleeding and tartar indicate disease activity that a routine cleaning cannot adequately address.
The area being treated is numbed with local anesthetic before the procedure begins. Many patients find the treatment more comfortable than they anticipated. Dr. Maddipati checks in with you throughout to ensure you feel at ease.
Specialized instruments — including ultrasonic scalers and hand instruments — are used to carefully remove plaque and hardened tartar from the root surfaces and within the pockets. The goal is to create a clean surface to which the gum tissue can respond positively.
The root surfaces of the teeth are carefully smoothed to make it more difficult for bacteria to re-adhere and to support the gum tissue in healing. This step is what differentiates a deep cleaning from a standard scaling.
In some cases, antimicrobial agents may be applied to the treated areas to support healing. Dr. Maddipati will discuss this with you if it is appropriate for your situation.
A re-evaluation appointment — typically scheduled after the tissues have had time to respond — allows Dr. Maddipati to measure pocket depths again and assess how the gums have responded. This helps determine whether additional treatment or a closer maintenance schedule is appropriate.
The number of appointments, the areas treated per visit, and any additional steps will vary based on the severity of disease and your individual clinical situation. Dr. Maddipati explains your personalized treatment plan — including a cost estimate — before any treatment begins.
After a deep cleaning, it is normal to experience some tenderness or sensitivity in the treated areas. The gums may also appear to look slightly different as swelling reduces and healthy tissue regenerates. These changes are a positive sign of healing.
The long-term outcome after periodontal treatment depends significantly on ongoing home care and regular professional maintenance visits. Dr. Maddipati will discuss a maintenance schedule appropriate for your situation at your follow-up evaluation.
