About This Treatment

Understanding Root Canal Treatment

Each of your teeth contains a soft inner space called the dental pulp — a collection of nerves, blood vessels, and connective tissue that helped the tooth develop. Once a tooth is fully formed, the pulp can be safely removed without affecting the tooth's function, because the tooth continues to receive nourishment from the surrounding tissues.

When bacteria enter the pulp — most commonly through deep decay, a crack or fracture, or repeated dental procedures on the same tooth — the pulp can become inflamed or infected. Root canal treatment (also called endodontic therapy) is the process of carefully removing this affected tissue, cleaning and shaping the root canals, and sealing them to prevent re-infection. The goal is to save your natural tooth and eliminate the source of discomfort.

Preserving your natural tooth is generally considered the most favorable outcome — it maintains the bone structure of your jaw, keeps neighboring teeth in their proper positions, and supports your ability to chew and speak comfortably. Dr. Maddipati will discuss all appropriate options with you at your evaluation.

What Leads to This?

Why Might the Pulp Become Affected?

Pulp involvement most often develops gradually. Tooth decay that is not addressed at an early stage can progress through the outer enamel and into the dentin — the softer layer beneath — and eventually reach the pulp. Once bacteria reach the pulp, inflammation and infection can follow.

Common Underlying Causes

  • Untreated tooth decay that has progressed deeply
  • A crack or fracture that creates a pathway for bacteria
  • Repeated dental procedures on the same tooth over time
  • A chip or injury that exposes the inner tooth
  • Severe wear from teeth grinding (bruxism)

Signs Worth Evaluating

  • Persistent or throbbing tooth discomfort
  • Sensitivity to heat or cold that lingers after the trigger is gone
  • Pain when biting or applying pressure
  • Swelling or tenderness in the gum near a tooth
  • Darkening or discoloration of a tooth
  • A small bump (sinus tract) on the gum near the tooth

Please note: These signs may have various causes. Only a clinical examination with appropriate X-rays can determine whether root canal treatment is appropriate for your situation. Dr. Maddipati will evaluate your tooth thoroughly and explain her findings clearly before recommending any course of action.

What to Expect

What Root Canal Treatment Involves

The steps below describe the general approach to root canal treatment. Every patient and every tooth is different — Dr. Maddipati will explain what your specific treatment will look like before anything begins.

1

Evaluation & Imaging

Dr. Maddipati takes X-rays of the tooth to assess the extent of any infection and the shape of the root canals. She reviews these images with you and explains what she sees in plain language.

2

Comfort Through Local Anesthesia

The area around the tooth is numbed thoroughly with local anesthetic before treatment begins. Most patients are pleasantly surprised by how comfortable the procedure is once the area is properly numbed.

3

Accessing & Cleaning the Canals

A small opening is made in the top of the tooth. Specialized instruments are used to carefully remove the affected pulp tissue and clean and shape the root canals. The canals are rinsed with antimicrobial solutions to help eliminate bacteria.

4

Sealing the Tooth

The cleaned canals are filled and sealed with a biocompatible material (gutta-percha) to prevent re-infection. The access opening is then sealed with a temporary or permanent filling.

5

Protecting the Tooth Afterward

Because root canal treatment removes the pulp, the tooth may become more brittle over time. In many cases, Dr. Maddipati recommends placing a dental crown to protect and strengthen the tooth. She will discuss this with you and explain why it may be beneficial for your specific tooth.

The number of appointments required and the steps involved may vary based on the complexity of the tooth, the extent of infection, and your individual clinical circumstances. Dr. Maddipati will outline your personalized treatment plan before beginning.

Your Options

Root Canal Treatment vs. Tooth Extraction — An Honest Comparison

When a tooth is severely infected or damaged, patients sometimes wonder whether it is better to save the tooth or simply have it removed. This is a completely reasonable question, and Dr. Maddipati will discuss both options openly with you based on your specific situation.

Preserving with Root Canal

  • Keeps your natural tooth in place
  • Supports healthy jawbone density over time
  • Neighboring teeth remain in their natural positions
  • Natural biting and chewing is generally maintained
  • No need for a replacement option (bridge or implant)

Tooth Extraction

  • May be appropriate if the tooth is not restorable
  • Once removed, neighboring teeth may gradually shift
  • Jawbone in the area can diminish over time without a tooth root
  • A replacement option (dental implant, bridge, or partial denture) is typically recommended to restore function
  • Dr. Maddipati will discuss all replacement options if extraction is the best path forward for you

Not every tooth can or should be saved. Dr. Maddipati will evaluate your tooth thoroughly and give you her honest assessment of what is possible and what she recommends — and why.

Frequently Asked Questions

These answers are for general educational purposes. Please schedule a consultation with Dr. Maddipati for guidance specific to your situation.

Root canal treatment is performed under local anesthesia, and most patients find the procedure far more comfortable than they anticipated. The discomfort associated with a deeply infected or inflamed tooth — before treatment — is generally much more significant than the treatment itself. Dr. Maddipati takes time to ensure the area is fully numb before beginning and checks in with you throughout the appointment.
In many cases, a dental crown is recommended after root canal treatment to protect and strengthen the tooth. Because the pulp has been removed, the tooth can become more brittle over time — a crown provides durable protection and helps maintain the tooth's function. Whether a crown is needed and what type is most appropriate depends on which tooth was treated and its current structure. Dr. Maddipati will discuss this with you as part of your overall treatment plan.
Texas Medicaid covers endodontic treatment on certain teeth for eligible adult and child patients. Coverage specifics — including which teeth are covered and any prior authorization requirements — depend on your individual plan. We verify your specific Medicaid benefits before scheduling any treatment and explain coverage clearly before we begin. Call us with your Medicaid information and we will confirm coverage for you.
Whenever it is clinically appropriate, preserving a natural tooth is generally considered the preferred option. Your natural tooth supports the surrounding jawbone, helps keep neighboring teeth in proper position, and provides a familiar and effective biting surface. That said, not every tooth can or should be saved — it depends on how much healthy tooth structure remains and the extent of any infection or damage. Dr. Maddipati will give you her honest clinical assessment and discuss all available options, including tooth replacement if extraction becomes the right path.
A tooth with an infected pulp will generally not resolve on its own. Without treatment, the infection may progress and affect the surrounding bone and tissues. Seeking a professional evaluation sooner rather than later typically gives you the most options. Dr. Maddipati will assess your situation and explain what she finds — including how urgent treatment is — so you can make an informed decision.
Root canal treatment is typically performed under local anesthesia, and most patients drive themselves home afterward without difficulty. If sedation is used, arrangements for a driver would be needed. Dr. Maddipati will discuss any specific post-treatment instructions with you at your appointment.
Some tenderness around the treated tooth is normal in the days following the procedure — this is your body's natural healing response. Over-the-counter pain relievers, taken as directed, are typically sufficient to manage any post-treatment soreness. Dr. Maddipati will provide specific aftercare instructions and let you know what to watch for.
The difference between a filling and root canal treatment depends on how deeply decay or damage has progressed — specifically, whether the pulp has been reached. A clinical exam and X-ray are the only reliable way to determine this. If you have been experiencing persistent or worsening tooth pain, sensitivity to temperature that lingers, or swelling near a tooth, it is worth scheduling an evaluation so Dr. Maddipati can assess the situation accurately.
