Over time, these acids gradually dissolve the mineral structure of tooth enamel — the hard outer layer of your tooth. This process of mineral loss is called demineralization, and when it progresses past the point where the tooth can naturally repair itself, a cavity forms.
Cavities are extremely common — they can develop at any age, in children and adults alike, and in patients who brush and floss regularly. This is not a reflection of poor hygiene; it reflects the complex interplay of diet, saliva, bacteria, and tooth structure. How Tooth Decay Progresses
Stage 1
Early Demineralization
A white spot may appear on the enamel surface — the earliest visible sign that acid is weakening the tooth. At this stage, the damage may be reversible with fluoride and improved oral hygiene. No drilling required.
Stage 2
Enamel Cavity
The decay has broken through the outer enamel. A filling is the appropriate treatment. Because enamel has no nerve endings, this stage is often painless — but that doesn't mean it should wait.
Stage 3
Dentin Involvement
Decay has reached the softer dentin layer beneath the enamel. Dentin contains tiny tubules connected to the nerve, which is why teeth at this stage often become sensitive to temperature or sweets. A filling is still typically appropriate.
Stage 4
Pulp Involvement
Decay has reached the inner pulp — the nerves and blood vessels inside the tooth. This stage often causes significant pain and may require root canal treatment to address the infection before a crown can be placed.

Dr. Maddipati reviews your X-ray with you, shows you the cavity, and explains the extent of the decay and what treatment involves. She answers any questions before anything else happens.

Comfortable Numbing
Local anesthetic is applied to the area. Most patients are surprised by how comfortable the appointment is once the tooth is numb. Dr. Maddipati gives the anesthetic time to work fully before proceeding.

Removing the Decay
The decayed portion of the tooth is carefully removed. Only the affected tooth structure is removed — Dr. Maddipati preserves as much healthy tooth as possible.

The composite resin is applied in layers, with each layer hardened using a curing light. The material is then shaped and polished to match the natural contour of your tooth and your bite.

Bite Check & Final Polish
Dr. Maddipati checks that your bite feels natural and comfortable. Any minor adjustments are made, and the surface is polished to a smooth finish that is easy to keep clean.

The area is fully numbed with local anesthetic before treatment begins, and most patients find the procedure very comfortable. You may feel pressure or vibration but should not feel sharp pain. After the appointment, once the anesthetic wears off, some mild sensitivity in the treated area is normal and typically resolves within a few days.
